Person Centred Tools

The use of Person Centred tools are at the cornerstone of how we deliver support as a service, prioritising a persons needs and preferences in their support plan. By identifying the person's unique strengths and abilities, these tools enable individuals to work collaboratively with their support team to develop strategies that promote greater autonomy, independence, and satisfaction in their daily life.

Below is a suite of tools that are available to anyone who wants them. They have a guide on how to use them and are beneficial to people who are looking to establish new supports and secure the ones that are working. Service providers are also encouraged to understand their use.

Person Centred tools have been around since the 1940’s and brought to prominence by Wolf Wolfensberger, a sociologist and researcher, in the 1970s.

    The "Anatomy of a Place" form can be used to assess the accessibility and usability of community spaces and venues, such as parks, restaurants, the beach, shops, workplaces, museums, and public transportation.

  • Pathway To Contribution

    The Pathway to Contribution tool is a resource designed to support individuals living with a disability to increase their involvement and engagement in their community. It is a structured process that helps to identify the individual's strengths, interests, and aspirations, and then creates a plan to support them in achieving their goals.

  • What's Working/ What's Not Working

    The What's Working/What's Not Working tool can be used to evaluate various aspects of a person’s support. It provides an opportunity for the individual to reflect on what is working well for them and what is not, to then adjust and improve their support plans and create effective strategies that improve engagement in various settings

  • Learning Log

    A learning log is a tool that can be used to track progress, record experiences, and identify areas for growth and development. In a disability setting, a learning log is used by individuals to document their goals, achievements, challenges, and learning experiences.

  • What's Working/ Not Working Multi Perspective

    This tool is particularly handy to draw out issues from multiple perspectives. It is a tool that enables people to identify where a tension point sits from different peoples' perspectives in each situation or context.
